No. DGMS/03-8/2026/CMC(HQ)] MINISTRY OF LABOUR AND EMPLOYMENT (Directorate General of Mines Safety) NOTIFICATION Dhanbad, the 10th July, 2026 G.S.R. 625(E).—In exercise of the powers conferred on me under sub-rule (1) of rule 173 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026, I, Ujjwal Tah, Chief Inspector-cum- Facilitator of Mines, also designated as the Director General of Mines Safety, hereby, specify the Forms for issue of Certificate of Training, as follows: Scope: Applicability: Forms for issue of Certificate of Training, as required under Rule 173(1) of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026. These Forms shall be applicable for issuance of certificate of training to every person who undergoes a course of training and completes the training followed by assessment and acceptable performance therein, under Rules 158, 159, 161 and/or 162 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026. Compliance Requirement: FORM – T(2) The form for the certificate of initial/refresher* training {See Rule 173(1) of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026} Certificate of Initial/Refresher* Training for employment in a mine on surface and in opencast workings/belowground degree I/II/III gassy coal/belowground metalliferous/oil mine* I, hereby certify that Shri/Smt/Miss S/o/D/o/W/o... ....................................Village .....................................Thana (police Station)... P.O...........................District...........................State...........................has duly undergone initial training from...........................to...........................as required under the provisions of Rule 158/ Rule 159/Rule 161* of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026, for employment in a mine on surface and in opencast workings/belowground degree I/II/III gassy coal/belowground metalliferous/oil mines*. After completion of training the trainee was assessed for his/her performance and found to be satisfactory.
