This form is to be given by a manager to the person to be examined, and such notice issued shall be sent to the examination authority. The notice requires the examinee to present themselves for a Periodical Medical Examination before a specified examining authority at a given place, date, and time. It explicitly states that failure to submit to the medical examination without reasonable cause will render the person ineligible for continuance in employment to the mine from a specified date. This notification is applicable to all mines.
