Scope: This form specify the notice of periodical medical examination to be given by manager to the person to be examined in case of he failed to attend medical examination for without any reasonable cause as per sub- rule 4 of the Rule 111 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026. Applicability: This form of the notice shall be issued by a mine manager to the person to be examined, in case of he failed to attend medical examination for without any reasonable cause and such notice issued shall be sent to examination authority by mine manager as per sub-rule 4 of the Rule 111 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026 and applicable to the all mines Second notice of Periodical medical examination No.........
