G.S.R. 604(E).βIn exercise of the powers conferred on me under sub-Rule 4 of the Rule 111 of the
Occupational Safety, Health and Working Conditions (Central) Rules, 2026, I, Ujjwal Tah, Chief Inspector-cum-
Facilitator of Mines, also designated as the Director General of Mines Safety, hereby, specify the form of the second
notice of medical examination shall be given by manager to the person to be examined, in case of he failed to attend
medical examination for without any reasonable cause.
Scope: This form specify the notice of periodical medical examination to be given by manager to the person to be
examined in case of he failed to attend medical examination for without any reasonable cause as per sub-
rule 4 of the Rule 111 of the Occupational Safety, Health and Working Conditions (Central) Rules, 2026.
Applicability: This form of the notice shall be issued by a mine manager to the person to be examined, in case of
he failed to attend medical examination for without any reasonable cause and such notice issued shall be sent to
examination authority by mine manager as per sub-rule 4 of the Rule 111 of the Occupational Safety, Health
and Working Conditions (Central) Rules, 2026 and applicable to the all mines

Son/daughter/wife of .......... failed to present himself/herself for a periodical medical examination on...... in

He/She is hereby again notified that he/she should present himself/herself for a periodical medical examination,
under sub-rule (4) 111 of The Occupational Safety, Health and Working Conditions (central) Rules, 2026,
before..........(give name of the examining authority) at ...... (give date) ........ at.......... (give time)
Shri/Shrimati/Miss......... ....., may note that if he/she fails to submit himself/herself for the medical examination
aforesaid, he/she shall not be retained in employment in the mine from.........(date)
